Upon arriving for each appointment, you will be greeted by our check-in team located at our reception desk in the waiting room. In order to make things run most smoothly please login to your patient portal account in advance of your initial evaluation to complete pre-registration forms. Be sure to arrive approximately 10 minutes early which will provide time to confirm your completed forms, photocopy your insurance card and identification, and have you change into appropriate clothing (if you haven't already done so) for your initial evaluation. Our facility is fully equipped with a locker room that has changing rooms and showers available for the convenience of all our patients.

Please complete and bring with you all of the following information. Please arrive at least 10 minutes early to ensure everything is processed and you are ready to get started!
- Complete New Patient Registration Packet located online within Patient Portal
- Complete Injury Assessment Form (specific to your injury)
- Bring your most current Health Insurance Card
- If you have one, bring your prescription from referring physician
- Bring any X-rays, MRIs or physician reports related to your injury (if applicable). (digital version preferred)
- MEDICARE Patients: Please also complete the Medicare ABN Form
What to wear:
Basic work-out/athletic attire is appropriate for physical therapy appointments. Below are more injury specific guidelines for what to wear specific to area of injury treatment:
- Low Back – Shorts and T-shirt
- Neck – Tank top
- Mid Back – T-shirt and shorts (sports bra for women)
- Upper Extremity – T-shirt
- Lower Extremity – Shorts
- Foot – Shorts, athletic shoes and previous orthotics (if applicable)
Your first visit will be your initial one hour evaluation in which you will meet your physical therapist and discuss your current condition or injury and identify your goals for pursuing physical therapy. At this time your physical therapists will conduct the appropriate tests to determine your range of motion, strength and integrity of your neurological system, in addition to hands-on tests to determine how your affected joints are moving and whether certain joints are contributing to your current dysfunction. Do not be surprised if your physical therapists investigates joints distant from your pain as some pain can be referred and we want to be sure to get to the root of the issue and not just treat your symptoms! At the end of the initial evaluation, your physical therapist will present his/her exam findings and present a recommended unique treatment plan to improve your condition. Every patient is different so it is extremely important to realize that treatment plans will be unique to you and variable from patient to patient. All of our therapists are open to discussion and questions. It is important at this initial evaluation that each patient walks away fully understanding and believing the future treatment plan is the right one for them!
Subsequent Treatment Sessions:
Each subsequent treatment will be approximately one hour to one hour and fifteen minutes in length. The maximize continuity (where schedules allow), we encourage each patient to remain with the same physical therapist throughout their course of treatment. We find this reflects a level of consistency for each and every treatment. There are exceptions, but we do our best to maintain continuity of care for the most effective outcomes.
